You need to clarify your question.
A disability plan replaces a portion of your income after you have been deemed too disabled to work for a certain length of time.
(Short-term disability plans might require a wait of 14 days while long-term disability plans may require you to be disabled for 60, 90 or even 180 days.) Some individual policies will pay regardless of where the injury occurred but most will not pay for on-the-job injuries or illnesses.

Are you injured or disabled? Are you trying to get your medical expenses for the injury covered? That's health or medical insurance.
In an "emergency," even medical plans with small, local provider networks will pay the claims of an out-of-area emergency provider, albeit you could pay significantly more than at a local preferred provider.

Did the injury occur on the job? (Workers Comp)

Disability insurance is not the same thing as medical (health) insurance.
I think you are confusing the two.

If you mean state disability insurance, you can only apply in the state where you work and live. If you have a private disabiity plan you have to ask them directly. If you are injured on the job while on company time, you get Workmans Comp.
