This will vary based on state, if the employees have beneficiaries (spouse and kids who are under the plan), and a few other factors.

For example, in NY, there is little insurance competition, so an individual would pay $1,000+ a month for their health insurance, if not in a group plan. I kid you not. If I had to guess the average nationwide, it would cost a few thousand each month.

However, keep in mind that employers of that size aren't required by law to have health insurance.

Depends on where you are and coverage. For halfway decent Blue Cross in CT the averages are 500 - 700/month for singles and 1500 - 1900/month for families. Yup you read that right. You can lower that (but not by a whole lot) by offering them HDHP (high deductible health plans) and an HSA (health savings accounts) into which they can contribute pretax wages for eligible expenses.
