What is my Insurance policy number?

My insurance card has the following numbers on it, but none of them seem to be the number I'm looking for.
It's MEGA Life Health Insurance, part of the First Health Network.
The numbers I have are:

RxBin: a 6 digit number
RxID: a 10 digit number
And
ID: An 11 digit number which is identical to the RxID, with a 0 appended to the front.

This insurance waiver is asking me for a Policy/Group number, as well as a Member/Sub ID.
Are both of these on my card?

I need to finish this today, but my insurance company is closed until Monday.

Please don't tell me you really fell for this scam company.

The policy number is the ID number. There will not be a Sub ID.

My suggestion is to find real health insurance before you actually have to use the insurance.

The policy number is the ID number. There will not be a Sub ID.

My suggestion is to find real health insurance before you actually have to use the insurance.
Your member/sub ID is the the Rx ID ##
The Policy/Group # should be on your card, iFnot, then look on your invoice.



These are the 4Rx items. BIN, PCN, Group,and your Member ID

Use the ID number. The group number should ALSO be listed on the front of your card.
