You are unaware of how labor intensive and time-consuming it is for a hospital (or any other provider, for that matter) to collect from private for-profit health insurance companies.
It wouldn't matter what type of identification that the patient held.

That said, my wife works in a Medicaid Fraud CU, and in our state over 93% of all verified cases of waste, fraud, and abuse have been perpetrated by providers, not "scamming" patients.
It's MUCH more likely that the hospital, itself, would be the entity attempting to bilk the government...

Gee...a national id card. Good at voting booths, the emergency room...when the govt wants to come get you.
