Does Canada and Mexico really have taxpayer funded healthcare for everyone or do people just misunderstand it?

Some of the questions on here seem to indicate that foreigners can walk into hospitals in these countries and get free healthcare.
Living in Australia, where we have a taxpayer funded healthcare system, this seems unlikely. Here, at least, you need to have a healthcare card, which I'm fairly sure is only available to citizens and possibly permanent residents (definitely not tourists) to claim anything. There are also generally charges if you have no booking, and not everything is covered.
On a side note, if you earn above a certain amount (above average but not well above - probably top third of earners) you have to either get private health insurance or pay extra tax.

Are Canada and Mexico really so generous to simply give free healthcare to anyone who walks in the door, or is this just exaggeration and misunderstandings?
